Qualifications

Responsibilities
- Deliver direct patient care, including bathing, dressing, grooming, toileting, and feeding.
- Assist with transfers, ambulation, and repositioning using safe lifting techniques.
- Monitor and report changes in patient condition to the Charge Nurse.
- Document patient care and daily activities per regulations.
- Support and follow individualized care plans to improve outcomes.
- Maintain a clean, safe, infection-controlled environment.
- Ensure medical equipment is clean, charged, and functioning.
- Participate in shift handoffs and team communications.
