• Conduct vital signs and report changes to skilled nursing staff.
• Help residents with transfers, ambulation, and mobility exercises.
• Assist with medication administration and other treatments as directed.
• Respond to resident calls and requests in a timely and professional manner.
• Collaborate with our interdisciplinary team to develop and implement individualized care plans.
