Job summary

Qualifications

Responsibilities

• Adheres to philosophy of supporting dignity, privacy, independence, choice, and a home-like environment.
• Receives and gives report from the previous shift.
• Reviews assignments with Designated Supervisor.
• Reads Resident records and maintains familiarity with service plans.
• Assists with ADLs and mobility while encouraging self-care.
• Provides services per Resident service plans, including bathing, toileting, transfers.
• Follows infection control procedures and maintains a safe environment.
• Notifies nurse of Resident status changes and records information.
