Qualifications

• Must hold CNA or Home Health Aide certification in the state.
• Experience in elder care preferred.

Responsibilities

• Review the logbook and assignment sheets for updates at the start of each shift.
• Assure that residents are neat, clean, and properly clothed. Assist with bathroom and hygiene needs.
• Tidy apartment; proactively assess potential safety concerns.
• Proactively assess potential safety concerns in common areas and report as needed.
• Transport residents to and from activities and dining.
• Provide SAMM assistance within the allotted time frame (if applicable).
• Accurate documents are provided, including SAMM timely (if applicable).
• Give a verbal report of shift change to the following shift.
• Mealtime duties include making morning coffee and serving breakfast; assist with setting tables and serving lunch/dinner; clean up and dishwashing duties.
• Provide one-on-one support to engage residents in activities or provide a quiet space to calm residents; facilitate activities.
• Demonstrate excellent customer service at all times.
• Commit to working with residents and families to develop and implement person-centered care strategies.
• Performs additional duties as assigned.
