• 18 years or older required.

Responsibilities

• Provide hands-on support with activities of daily living including personal hygiene, dressing and grooming.
• Deliver resident-centered care per individualized service plans and maintain documentation.
• Monitor resident well-being through routine check-ins and meaningful daily interactions.
• Assist with mobility, transfers, dining support and safe use of adaptive equipment.
• Communicate observations and changes in resident health or behavior to the nursing team.
• Build positive relationships with residents and support individualized care.
