Job summary

Hazelhurst Court Care & Rehab is hiring a Certified Nurse Assistant (CNA) under new pay rates to join their care team in Hazlehurst, GA. The CNA will support short term rehabilitation and long-term care residents under the supervision of the Director of Nursing.

Responsibilities include direct personal care, patient mobility, daily living assistance, and maintaining accurate records in the EMR. The role emphasizes compassionate, compliant care in a skilled nursing setting.

Qualifications

• A minimum of a high school education or high school equivalency diploma is preferred for this position.
• Candidate must read, write and understand the English language.
• Must complete at least 18 hours Continuing Education to maintain Certification.
• Must have completed a State approved Certified Nursing Assistant program.

Responsibilities

• Performs duties in accordance with established methods and recognized standards.
• Provides maximum customer care services within delegated tasks.
• Assists with admission, discharge and transfer of customers; assist nurse with treatments and personal belongings.
• Bathes customers in bed, tub or shower, cleans and cuts fingernails and provides shampoos.
• Positions customers in correct body alignment and applies safety measures under supervision.
• Assists customers in/out of bed, walking and transports via wheelchair/stretcher; may accompany to appointments.
• Coordinates scheduling for OT/PT/ST to ensure timely access to therapy.
• Accompanies customers to appointments as needed.
• Participates in formulation and follow-through of Nursing Care Plan.
• Takes and records weights, vital signs in EMR.
• Gives skin care treatments and applies dressings under direction of nurse.
• Feeds customers and assists with meals; provides nourishment and hydration.
• Answers call lights, delivers messages, and assists with bedpans/urinals.
• Performs isolation care as required and reports changes to Nurse/Supervisor.
• Cleans and sterilizes customer equipment and manages linen as needed.

Education

High school diploma or equivalent

English reading/writing proficiency

18 hours CE for CNA certification

State approved CNA program completion
