Job summary

Haven Hospice in Lake City, FL is seeking a compassionate Hospice CNA for PRN night shifts (7p-7a). You will provide 1:1 personal care, monitor patients, and report changes while working with a supportive interdisciplinary team.

The role offers flexible scheduling, less time on your feet than other settings, and opportunities for advancement. Qualifications include state training requirements, at least six months of personal care experience, 18+ years old, and CPR certification.

Qualifications

• Meet the state training requirements and provide personal care.
• Minimum six months' experience including personal care.
• Must be 18+ years old with CPR certification.
• Able to read and follow written instructions and document care given.

Responsibilities

• Provide 1:1 personal care including bathing, dressing, toileting and mobility.
• Monitor patient condition and report changes to the team.
• Assist with ambulation, transfers, ROM and safe equipment use.
• Assist with medication administration as directed by state regulations.
• Offer emotional support and companionship to patients and families.
• Perform light housekeeping and meal assistance.
• Maintain accurate, timely documentation and collaborate with the hospice interdisciplinary team.

As a Hospice CNA You will:

• Personal Care : Help with bathing, dressing, toileting, mobility, and repositioning
• Monitor & Report: Observe and report changes in the patient’s condition
• Mobility : Assist with ambulation, transfers, ROM exercises and safe equipment use
• Medication Assistance : Help with self-administered medications per state regulations
• Emotional Support: Offer companionship and comfort to patients and families.
• Light Housekeeping: Change linens, assist with meals, and maintain a clean space.
• Documentation: Maintain accurate and timely documentation
• Team Participation : Collaborate with hospice interdisciplinary team

Qualifications

• Meet the training requirements of the state of practice
• Minimum of six months experience including providing personal care
• Must be 18+ years of age
• Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
• Possess and maintain current CPR certification
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill
• Ability to work with little direct supervision
• Ability to handle death/dying
• Strong oral and written communication skills
• Good Organizational Skills
