Job summary

At Home Care Hospice seeks a compassionate Hospice CNA to provide dignified, in-home care to patients in Fredericksburg and surrounding areas. You will work a full-time schedule Monday–Friday, 8 AM–5 PM, delivering personal care and emotional support with independence and team collaboration.

The role includes patient mobility assistance, medication support, documentation, and ongoing communication with the hospice interdisciplinary team. State requirements and CPR certification are necessary.

Qualifications

• Meet the training requirements of the state of practice.
• Minimum of six months experience including providing personal care.
• Must be 18+ years of age.
• CPR certification required and up-to-date.
• Ability to read and follow written instructions and document care given.
• Comfortable providing hospice care to terminally ill patients.

Responsibilities

• Provide 1:1 patient care including bathing, dressing, toileting, mobility and repositioning.
• Monitor and report changes in patient condition.
• Assist with ambulation, transfers and ROM exercises.
• Assist with medications per state regulations.
• Offer emotional support and companionship to patients and families.
• Light housekeeping and meal assistance.
• Maintain accurate and timely documentation.
• Collaborate with hospice team.

Responsibilities

As a Hospice CNA You will:

• Personal Care : Help with bathing, dressing, toileting, mobility, and repositioning
• Monitor & Report: Observe and report changes in the patient’s condition
• Mobility : Assist with ambulation, transfers, ROM exercises and safe equipment use
• Medication Assistance : Help with self-administered medications per state regulations
• Emotional Support: Offer companionship and comfort to patients and families.
• Light Housekeeping: Change linens, assist with meals, and maintain a clean space.
• Documentation: Maintain accurate and timely documentation
• Team Participation : Collaborate with hospice interdisciplinary team

Qualifications

• Meet the training requirements of the state of practice
• Minimum of six months experience including providing personal care
• Must be 18+ years of age
• Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
• Possess and maintain current CPR certification
• Ability to read and follow written instructions and document care given
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill
• Ability to work with little direct supervision
• Ability to handle death/dying
• Strong oral and written communication skillsGood Organizational Skills
