Good Samaritan Society in Wagner, SD is seeking a CNA to provide resident-centered nursing care and daily living assistance under the supervision of an RN. You will support infection control, help with activities of daily living, vital signs, and documentation, while participating in wellness programs and care planning.

The role requires CNA certification or enrollment, a high school diploma or GED, and commitment to ongoing competency testing and CE credits.

Qualifications

• CNA certification required or in training with provider support.
• High school diploma or GED preferred.
• Prior clinical or nursing assistant experience in long-term care preferred.
• Annual competency testing and 12 hours of CE credits required.

Responsibilities

• Provide resident-centered nursing care and daily living assistance under RN supervision.
• Deliver age-appropriate care per the care plan.
• Follow infection prevention policies and PPE guidelines.
• Assist with bathing, toileting, grooming, dressing, vital signs, and psychosocial support.
• Assist transfers, repositioning, walking, and ROM exercises.
• Assist with meals, dining, and hydration as appropriate.
• Document resident interactions and outcomes related to care, behavior, activity, and dietary needs.
• Communicate changes in resident condition to the charge nurse.
• Collect non-invasive specimens and take vital signs.
• Provide housekeeping and laundry to maintain clean living areas.
• Support wellness and recreational programs and participate in committees.

Responsibilities

• Provide resident-centered nursing care and daily living assistance to assigned residents under the supervision of a registered nurse (RN).
• Deliver age-appropriate care related to the physical and psychological needs of the resident as per the care plan.
• Follow infection prevention and control policies and use of Personal Protective Equipment; adhere to facility standards.
• Assist with basic health care needs including bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support.
• Assist residents in transferring, repositioning, and walking using correct techniques and equipment; provide range of motion and passive exercises.
• Assist with food preparation and dining to create a positive dining experience; help with meals and snacks and provide water and supplements as appropriate.
• Assist providers and nursing staff with procedures as needed; document resident interactions and outcomes related to care, behavior, activity, and dietary needs.
• Communicate changes in the resident's condition and care-related concerns to the charge nurse.
• Collect non-invasive body fluid specimens when required and take vital signs.
• Provide housekeeping and laundry services to maintain clean and operational living areas.
• Support wellness and recreational programs and encourage resident participation.
• Participate in departmental or organizational committees and task forces as needed.

Qualifications

• High school graduate or GED preferred, but not required.
• Minimum age 16; prior clinical or nursing assistant experience in long-term care preferred.
• Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state-certified CNA within four months of hire (enrolled in a training program or in the process of certification through Endorsement). CNA training and certification provided by the facility.
• Completion of yearly department-specific competency testing and maintenance of continuing education credits (minimum 12 hours annually) and department-specific certifications.
• In applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry when required.
• May classify as a Sanford Category II Driver requiring a motor vehicle report and proof of valid driver’s license; must be medically qualified to perform essential driving functions per Sanford Driving Policy upon leadership request.
