Job Summary
- Provides resident‑centered nursing care and daily living assistance to assigned residents under the supervision of a registered nurse.
- Delivers age‑appropriate care related to the physical and psychological needs of residents per individual care plans.
- Follows infection prevention and control policies and personal protective equipment use.
- Assists with basic health‑care needs such as bathing, toileting, grooming, dressing, obtaining and recording vital signs, and providing psychosocial support.
- Utilizes correct transfer techniques and equipment for transferring, repositioning, and walking residents, and performs range‑of‑motion and passive exercises.
- Completes food‑preparation and dining tasks, creating a positive dining experience for residents.
- Assists residents with meals and snacks, provides water and supplements as appropriate.
- Aids nursing staff and providers with procedures when needed.
- Documents resident interactions and outcomes related to care, behavior, activity, and dietary needs.
- Communicates changes in a resident’s condition and concerns to the charge nurse.
- Collects non‑invasive body‑fluid specimens and records vital signs.
- Provides housekeeping and laundry assistance to maintain resident living areas.
- Supports wellness and recreational programs, encouraging resident participation.
- May participate in departmental or organizational committees and task forces as needed.
Qualifications
- High school graduate or GED preferred (not required).
- Minimum age 16.
- Prior clinical or nursing assistant experience in long‑term care preferred.
- Certified Nursing Assistant (CNA) certification required with the State Board of Nursing (or state‑certified CNA within four months of start date by enrollment in a training program or in the process of obtaining certification through endorsement).
- CNA training and certification is required and provided by the facility.
- Completes yearly department‑specific competency testing and maintains a record of continuing‑education credits, with a minimum of 12 hours annually.
- Maintains all department‑specific required certifications.
- Where applicable, is certified and registered with the Unlicensed Assistive Person (UAP) Registry.
- May be classified as a Sanford Category II Driver, requiring a valid driver’s license and medical qualification to perform essential driving functions (if responsibilities apply).
