Job summary

Patriot Health and Rehabilitation Center in Paris, TN is seeking compassionate Certified Nursing Assistants (CNAs) to join our skilled nursing team. Per Diem or PRN roles involve direct resident care, monitoring vital signs, and supporting daily activities in a fast-paced long-term care setting.

Active Tennessee CNA certification is required, with experience in long-term care preferred. Excellent communication and teamwork are essential as you collaborate with nursing staff to ensure resident

Qualifications

• Active Tennessee CNA certification required.
• Experience in long-term care, skilled nursing, or rehab preferred.
• Excellent communication and teamwork abilities.
• Willingness to work weekends and holidays as needed.

Responsibilities

• Provide direct professional care to residents, including assistance with activities of daily living.
• Assist residents with mobility, transfers, and exercise programs.
• Measure and record vital signs, weight and health indicators.
• Observe residents' condition and report concerns to supervisors.
• Support nutritional needs by assisting with meals, hydration and feeding as needed.
• Maintain a safe, clean environment, including changing linens.
• Assist with admissions, discharges and transfers.
• Document care provided accurately in medical records.
• Communicate with residents, facilities and team members.
• Respond promptly to call lights and urgent needs.

Per Diem or PRN | Skilled Nursing / Long-Term Care

Patriot Health and Rehabilitation Center is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to join our growing team in Paris, Tennessee. In this role, you will work closely with our nursing staff to provide excellent care to our residents, assisting with daily activities, monitoring vital signs, and ensuring their comfort and well-being.

CNA Requirements

• -Active CNA Certification in Tennessee
• -Experience in long-term care, skilled nursing, or rehab preferred
• -Excellent communication and teamwork abilities
• -Ability to prioritize and work in a fast-paced environment
• -Willingness to work weekends and holidays as needed

CNA Responsibilities

• -Provide direct professional careto residents, including assistant with activities of daily living
• -Assist residents with mobility, transfers, and exercise programs
• -Measure and record vital signs, weight, and other health indicators
• -Observe residents' physical and emotional condition and report any concerns to supervisors
• -Support residents' nutritional needs by assisting with meals, hydration and feeding as needed
• -Maintain a safe and clean environment, including making beds and changing linens
• -Assist with resident admissions, discharges and transfers while ensuring comfort
• -Document care provided accurately in medical records and charts
• -Communicate effectively with residents, facilities and other team members
• -Respond promptly to resident call lights and urgent needs
