Quality Private Home Care Services is seeking compassionate CNAs/caregivers to provide in-home care for patients in the Boston, MA area. You will work closely with a medical team to monitor patient conditions, perform personal care tasks, and support families in maintaining daily routines.

The role requires a high school diploma, CNA certification/licensure, and excellent verbal and written communication skills.

Responsabilités

• Enable patients to stay in their homes by monitoring and recording condition, providing support and personal services.
• Monitor patient condition by observing physical and mental state, intake/output, and exercise.
• Provide light housekeeping, laundry, shopping for essentials, meal prep and errands.
• Assist patients with bathing, dressing and grooming.
• Teach patients or families to use cane, walker, utensils or hygiene techniques.
• Educate family on safe lifting and repositioning of patient.
• Advise on nutrition, cleanliness and housekeeping.
• Record patient information in the portal and notify nurse supervisor of changes.
• Maintain a safe, secure, and healthy home environment by following policies and standards.
• Protect the agency by adhering to professional standards and regulatory requirements.
• Seek opportunities to add value to patient care and service quality.

• Enables patients to stay in their homes by monitoring and recording patient condition, providing support and personal services, and teaching families.
• Monitors patient condition by observing physical and mental condition, intake and output, and exercise.
• Supports patient by providing light housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands.
• Assists patients by providing personal services, such as, bathing, dressing, and grooming.
• Helps patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene.
• Helps family members care for the patient by teaching family members appropriate ways to lift, turn, and re-position the patient.
• Advises on nutrition, cleanliness, and housekeeping.
• Records patient information by making entries in the patient portal and notifying nursing supervisor of changing or unusual conditions.
• Maintains a safe, secure, and healthy patient environment by following standards, and procedures, maintaining security precautions, and following prescribed dietary requirements and nutrition standards.
• Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements.
• Enhances service reputation by accepting ownership, for accomplishing new and different requests, and exploring opportunities to add value to job accomplishments.

Required:
