Job summary

Responsibilities include observing patients, taking vital signs, assisting with transfers, documenting in medical records, de-escalation, and coordinating with the care team to promote rehab goals.

Qualifications

• High school diploma or equivalent required.
• Certified Nursing Assistant (CNA) certification or license required.
• Experience in behavioral health setting preferred.

Responsibilities

• Provide patient care and ensure safety under supervision.
• Take vital signs and document results accurately.
• Assist with transferring, turning, and mobility.
• Engage patients in activities and de-escalate distress.
