Transitions Healthcare Washington PA is seeking compassionate CNAs to join our nursing team and provide high-quality resident care. Full-time roles are available and applicants must be eligible to work in Pennsylvania with PA certification or a multi-state credential.

Formación

• Current Pennsylvania CNA certification required, or eligibility to work in PA with an Ohio/West Virginia credential.
• CPR certification preferred.
• Experience in long-term care, skilled nursing, rehabilitation, or hospital settings is preferred but not required.

Responsabilidades

• Provide resident-centered care including bathing, dressing, grooming, toileting, feeding, oral hygiene and mobility.
• Assist with repositioning, transfers, and ambulation while promoting comfort and independence.
• Document vital signs, weights, intake/output, and other observations.
• Collect routine specimens as directed.
• Perform restorative nursing activities per care plans.
• Report changes to licensed nursing staff promptly.
• Respond to call lights and resident requests.
• Assist with admissions, transfers, and discharges.
• Maintain clean, safe resident rooms and common areas.
• Document care in electronic medical record.
• Communicate with residents, families, visitors, and healthcare team.
• Follow infection control and safety policies.
