CHI Health Schuyler is seeking a Certified Nursing Assistant to support patient care under the supervision of the RN. You will help with basic nursing tasks, assist with patient transport, and participate in the patient's plan of care as part of the health care team.

Job Summary and Responsibilities

As a Certified Nurse Assistant (CNA), you will be a cornerstone of patient care, directly contributing to comfort, safety, and recovery by providing essential support under RN direction. • Under the direction and supervision of the Licensed Practitioner, RN or LPN, participates in the nursing process which includes: assess, plan, implement, and evaluate. The nursing process is an ongoing process that can end at any stage.
• Assessment - Contributes to the assessment of the health status of the healthcare consumer.
• Plan - Contributes to the plan of care through observation, data collection and communication of findings to the appropriate member of the multidisciplinary team.
• Implement - Provides basic patient care activities based on the plan of care and as delegated.
• Evaluate - Contributes to the evaluation of the healthcare consumer by observing responses to interventions and communicating findings to the appropriate member of the multidisciplinary team.
• Documents cares provided in the healthcare consumer's record.
