Job description

Purpose of Position

The primary purpose of this position is to provide each assigned resident with routine daily personal care and service in accordance with current federal, state, and local standards, guidelines and regulations, and as may be directed by the administrator, to assure that the highest degree of care and/or services can be maintained at all times.

Delegation of Authority

As Certified Nurse Aide, the employee is delegated the administrative authority, responsibility, and accountability necessary for carrying out assigned duties.

Job Functions

This description serves as a guide for job expectations and outlines the essential functions. It is not exhaustive; additional duties may be assigned by supervisors as deemed appropriate.

Support of Mission and Philosophy

Employees must foster an atmosphere that reflects the Mission and Philosophy of the Carmelite Sisters for the Aged and Infirm, incorporating the Philosophy Values into daily care and interactions with residents, co‑workers, volunteers, and guests.

Essential Job Functions
Direct Care
  • Knock before entering a resident’s room and identify residents before providing care, serving meals, etc., as necessary.
  • Daily review and follow ADL care plan directives and address questions with the Team Leader and/or RN Supervisor.
  • Provide care efficiently and as required by the resident’s ADL care plan, including but not limited to:
    • Bed mobility, transfers, and repositioning
    • Bathing
    • Dressing
    • Grooming, shaving male residents, and maintaining clean shaven female residents
    • Nail care
    • Eating assistance
    • Toileting and incontinence care (perineal care, catheter care, skin integrity)
    • Hearing aids, glasses, denture, and mouth care
    • Transport to and from appointments/activities
  • Ensure all nursing care is provided in privacy.
  • Prepare residents for meals – take to bathroom, wash hands, comb hair, ensure dentures are in place, raise bed, position tables, and place garment protectors.
  • Serve food trays, record residents’ food/fluid intake, and report changes in eating habits.
  • Keep water pitchers clean and filled with fresh water on each shift.
  • Serve between‑meal and bedtime snacks; provide hydration as required or directed.
  • Perform after‑meal care – remove trays, clean hands, face, clothing; brush teeth; clean dentures.
  • Maintain residents dry by changing gown, clothing, and linen when wet or soiled.
  • Change bed linens and make beds (occupied and unoccupied).
  • Position residents per care plan and turn bedfast residents at least every two hours as planned.
  • Check and report bowel movements and stool characteristics.
  • Collect urine specimens and provide daily indwelling catheter care.
  • Assist residents to walk with or without aid devices, perform restorative procedures, document as required, and provide daily range‑of‑motion exercises.
  • Weigh residents and document weight on proper forms.
  • Keep the nurse call system within easy reach and answer calls promptly.
  • Conduct rounds as required and ensure residents who cannot call for help are checked frequently.
  • Check each resident routinely to confirm personal care needs are met in accordance with wishes and care plan.
  • Observe and report pressure areas and skin breakdowns and injuries of unknown source, including skin tears.
  • Assist residents in preparing for activity and social programs and assist with activities as requested or assigned.
  • Provide residents with reality orientation as instructed.
  • Watch for changes in room temperature, ventilation, lighting, etc., and report as needed.
  • Assist with the care of and provide comfort to the dying resident, including post‑mortem care as instructed.
  • Treat all residents fairly and with kindness, respecting rights and dignity and allowing choice, including refusing care or treatment.
  • Maintain confidentiality of resident and facility information and comply with HIPAA and other applicable laws.
  • Provide a safe, secure, home‑like environment, exercising care and concern for facility property and residents’ belongings.
Communication
  • Participate in providing the nursing report upon arrival and departure of duty.
  • Effectively communicate and use appropriate interventions with residents who have behavioral health issues, including dementia.
  • Report all accidents, incidents, changes in resident condition, complaints, grievances, and allegations of abuse, neglect, or misappropriation of property to the Team Leader/RN Supervisor.
  • Immediately notify the Team Leader/RN Supervisor of any resident missing from the facility.
  • Attend all staff meetings and/or review minutes from staff meetings.
Documentation
  • Complete required documentation once care, treatment, or restorative program is provided.
  • Record all documentation timely and accurately, legibly and accurately.
  • Maintain bowel and bladder, weight, intake and output records as instructed.
Safety/Procedures
  • Follow established policies concerning exposure to blood/body fluids and infection control measures.
  • Check rooms for food articles and report improper storage.
  • Follow established infection control measures, hand hygiene, and standard precautions.
  • Follow transmission‑based precautions as required.
  • Follow policies for the use and disposal of personal protective equipment.
  • Keep floors dry and report spills immediately.
  • Store excess supplies and equipment off the floor and in designated areas.
  • Clean, disinfect, and return all resident equipment after use.
  • Follow smoking regulations and report violations.
  • Maintain compliance with facility policies, procedures, and applicable regulations.
  • Maintain professional demeanor and appearance, consistently complying with the code of conduct and appearance and hygiene policy.
  • Promote collaborative relationships and teamwork, respecting all individuals.
  • Train or direct other employees and volunteers as requested.
  • Attend and participate in in‑service education, department meetings, and mandatory training per the position’s requirements.
  • Respond to fire alarms per the Fire Safety Manual and report all safety violations.
  • Use only equipment trained to use and report any special needs.
  • Follow regulations governing safety data sheets for hazardous chemicals.
  • Use personal protective equipment as required.
  • Follow ergonomics policies for lifting and repetitive tasks.
  • Report unsafe or hazardous conditions immediately to the supervisor.
  • Use supplies efficiently to avoid waste.
  • Participate in Quality Assurance/Performance Improvement initiatives and offer suggestions to improve resident care.
  • Review corporate compliance materials and report issues per established procedures.

Education
  • Minimum of a high school diploma or equivalent.
  • Completed a nurse aide training program approved by the Pennsylvania Department of Education, or currently enrolled with the Pennsylvania Department of Health as an eligible nurse aide.
Experience
  • On‑the‑job training is provided for those who meet the education requirements.
  • No substantiated findings of abuse, neglect, or misappropriation of property on file with the Pennsylvania Nurse Aide Registry.
Certificates & Licenses
  • Must be certified as a nurse aide by the Pennsylvania Department of Health. If not certified at time of hire, must complete certification within 120 days of employment.
Physical/Sensory Requirements
  • Must be able to move intermittently throughout the work day.
  • Must read, write, speak, and comprehend English in an understandable manner.

  • Must see, hear, or use prosthetics that enable adequate function for the position.
