Job summary

College of Pastoral Supervision and Psychotherapy seeks a dual MOA/clinical role responsible for patient intake, documentation, and care coordination. The person will obtain vital signs, maintain health records, and assist in care planning to minimize medication errors.

They will triage calls, verify insurance, and navigate the EMR to support efficient patient flow. The position requires an NC LPN license or CMA certification with at least one year of clinic experience; Family Medicine

Qualifications

  • Valid NC LPN license or CMA certification.
  • Minimum of one year of work experience in a medical clinic or similar environment.
  • Preferred, but not required, Family Medicine experience.

Responsibilities

  • Plan services using an appropriate problem-solving approach.
  • Obtain vital signs and health information for chart preparation.
  • Maintain up-to-date patient health records, including pain assessment and current medications.
  • Ensure documentation is complete, concise, and correct.
  • Assist in care planning to reduce medication errors on refills.
  • Triages patients over the phone and documents data clearly in the EMR.
  • Promote patient self check-in and maximize use of MyChart where possible.
  • Navigate the EMR efficiently to support care management.
  • Support continuity of care with timely notes and follow-up.
  • Answer phones, route calls, and manage messages with urgency in distress situations.
  • Triages basic patient care needs via telephone encounters.
  • Schedule appointments and register new patients with accurate demographics and insurance info.
  • Verify insurance eligibility.
  • Check patients in/out with proper ICD-10 and CPT coding.
  • Refill medications per guidelines.
  • Maintain patient satisfaction and excellent customer service.
  • Assist patients with navigating MyChart and finding results.
  • Monitor program digital platforms (FHOTG, OAH, Virtual provider, etc.).
  • Provide patient education per provider instructions.
  • Record and report information accurately to providers.

Overview

This role will function as both medical office assistant (MOA) and clinical, to include all duties and responsibilities for those roles. In addition to the following essential position competencies, other competencies may be required to meet changing organization needs.

Responsibilities
  • Uses an appropriate problem-solving approach to plan services.
  • Obtain vital signs and general health information for patients' chart and prepare patients for examination.
  • Maintains patient health records in an up-to-date status especially regarding pain assessment and current medication sheet.
  • Assures documentation is complete, concise, and correct.
  • Contributes to plan of care for individuals and groups of patients utilizing all principles to reduce medication errors when calling in medications or refills.
  • Demonstrates initiative in suggesting and implementing problem solving.
  • Triages patients over the phone and enters accurate and concise data into the EMR while paying attention to spelling and clarity.
  • Promotes E-Check-in as much as possible and strives for 80% or better of patients using this feature.
  • Navigates the EMR System in an efficient manner.
  • Facilitates quality care management through best practice utilization of the clinic EMR.
  • Promotes continuity of care seen by appropriate actions notes and prompt follow up.
  • Answers phones, routes calls and/or takes accurate message. Demonstrates a sense of urgency relating to a patient's level of distress.
  • Triages basic patient care needs, generating a telephone encounter depending upon the situation. Assure a duplicate message has not already been started.
  • Manages the scheduling of patient's appointments. Registers new patients and maintains current and accurate up-to-date demographic and insurance information for all patients. Performs according to policy regulations associated with Privacy Notification, Medicare secondary, signed demographic registrations, consent to treat minor, etc.
  • Works closely with program paramedic to assure all program patients are seen in a timely and coordinated manner.
  • Verifies insurance eligibility.
  • Checks patients in and/or out for visits appropriately. Accurate and appropriate use of ICD-10 and CPT codes.
  • Recognizes how fraud and abuse interplay into daily roles.
  • Refills medication according to medication refill guidelines.
  • Sensitive to patients' satisfaction as it pertains to customer service-people skills in all clinical situations.
  • Having patience to help patients with navigating MyChart, finding documents to sign, and where to find results, work notes, etc.
  • Responsible for monitoring all program digital platforms, such as FirstHealth-On-the-Go (FHOTG), Observation at Home (OAH), Virtual provider, and other projects as program expands.
  • Provides services with consideration of the recipient’s needs.
  • Ensure the patient is checked in and waiting on the provider virtually in a timely manner.
  • Assist the provider in communicating with other staff about swabs, or blood work that needs to be drawn.
  • Track all labs and test for outcomes and follow-up.
  • Identifies clinical significance of data and informs provider appropriately and provides follow-up with accurate documentation.
  • Teaches/directs/advises/informs others in an appropriate manner.
  • Serves as a communication link between the patient and the provider.
  • Accurately and clearly communicates data to the appropriate healthcare provider in a timely manner.
  • Assist with scheduling and preparing follow-up appointments as needed.
  • Teaches patients according to provider’s instructions and in conjunction with age-appropriate guidelines.
  • Utilizes EMR to support patient education and documents accordingly.
  • Reports/records information appropriately.
  • Enters and records patient information in E MR in a concise and accurate manner.
  • Ensures that all test results, interpretations, etc. are received in patient E MR in a timely manner.
  • Uses approved format for recording and reporting messages, i.e., telephone encounter, etc.
  • Maintains written reports of all situations requiring the manager’s attention.
  • Responds to the follow-up action plans sent by providers goaled at achieving maximum level of care continuity in preparation for designated patient appointments. - Example: Remind patient to have a log of blood pressures at appointment, etc.
Qualifications

Must have a valid NC LPN License or CMA Certification. Minimum one year of work experience in a medical clinic or similar environment. Preferred, but not required, Family Medicine work experience.
