Job summary

Hartford HealthCare’s Rushford Center is seeking a skilled clinical professional to coordinate and deliver evidence-based behavioral health services. You will work with interdisciplinary teams to assess needs, develop care plans, and provide direct patient care across the continuum of care.

The role emphasizes ethical practice, collaboration, and education for patients and families, with opportunities to contribute to program development and hospital-wide initiatives.

Qualifications

  • Master's degree in Social Work, Counseling, or Marriage and Family Therapy or related field.
  • LMSW, LPC-A, LMFT-A, or CADC.
  • Valid Driver's License.

Responsibilities

  • Conducts a comprehensive bio-psychosocial assessment and establishes a care plan.
  • Collaborates with the interdisciplinary team to assess level of care and coordinate referrals.
  • Utilizes therapeutic methods in line with the care plan.
  • Monitors progress and revises plans to meet needs.
  • Provides education to patients and families on goals, disease prevention, and treatment adherence.
  • Performs direct patient care including case management, care coordination, and crisis intervention as needed.
  • Refers patients to appropriate care levels and resources.
  • Maintains timely, patient-centered documentation as required by licensing and agencies.
  • Participates in administrative and development activities and team meetings.
  • Supports hospital/unit programming to meet clinical and strategic needs.
  • Works as a team member to improve patient experience and resource utilization.

Job Summary

Works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.

Job Responsibilities
  1. Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.
    1. Collaborates with the interdisciplinary Team to assess for appropriate level of care and risk factors; develops and implements care/discharge plan and coordinates consults/referrals to remove barriers and promote optimal clinical and financial outcomes.
    2. Utilizes specific therapeutic methods/strategies/interventions in accordance with individualized care plan.
    3. Monitors progress of interventions and revises plan to meet patient/family needs.
    4. Provides education to patient/family with respect to meeting established goals including but not limited to disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens.
  2. Provides direct patient care that may include: case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end-of-life care, referral and discharge planning.
  3. Refers patient to appropriate levels of care and available resources that promote the individualized care plan.
  4. Maintainstimely patient-centered documentation thatreflect the patient and client systems’ pertinent information for assessment, interventions and outcomesin accordance with regulatory licensing requirements and third-party agencies.
  5. Participates in administrative and development activities including team meetings, clinical supervision of staff and staff education.
  6. Supports the development of hospital and/or unit-based programming to meet the clinical and strategic needs of the patients and the milieu.
  7. Works as a team member to contribute to improved patient experience.
  8. Ensures the cost-effective utilization of resources.
  • Master's Degree in Social Work, or Counseling, or Marriage and Family Therapy or Related Human Service Field
  • Previous experience in a healthcare setting
  • LMSW, LPC-A, LMFT-A, or CADC
  • Valid Driver's License
  • Utilize supervision and team approaches in the context of patient assessment and clinical interventions
