Job summary

HCRG Care Group is seeking a Clinical Triage Practitioner to support Children's Specialist Services across Bath & North East Somerset, Swindon and Wiltshire. You will triage referrals, provide clinical advice, and help coordinate care for children and families within a multidisciplinary team.

Applicants should be registered (NMC/HCPC), with at least 3 years post-registration experience, and a solid understanding of safeguarding and child protection processes. Travel across the region is required.

  • Work with health, education, social care and third-sector organisations to coordinate delivery.

Job description

An exciting opportunity has arisen for a Clinical Triage Practitioner to support the delivery of Children's Specialist Services across Bath & North East Somerset, Swindon and Wiltshire. The post holder will undertake clinical triage of referrals, including urgent referrals, ensuring timely assessment, decision-making and communication of outcomes. Working as part of a multidisciplinary team, the successful candidate will support complex referral discussions, provide clinical advice and contribute to effective care coordination across services. The role requires close partnership working with healthcare professionals, education services, voluntary sector organisations and families to ensure children and young people receive appropriate, safe and effective support through a coordinated and integrated care pathway.

  • Undertake clinical triage of referrals into Children's Specialist Services and School Aged Health Services in accordance with agreed pathways and timescales.
  • Support the Clinical Triage Lead in the delivery of multidisciplinary triage for complex and multiple-need referrals.
  • Provide specialist clinical advice and support to Care Coordination and Single Point of Access functions.
  • Ensure children, young people and families are directed to appropriate services through effective care navigation.
  • Communicate complex, sensitive and potentially distressing information to families and professionals as required.
  • Work collaboratively with health, education, social care and third-sector organisations to ensure coordinated care delivery.
  • Identify, escalation and manage safeguarding concerns, clinical risks and pathway issues.
  • Contribute to service development, governance processes and implementation of standard operating procedures.
  • Registered Nurse or Allied Health Professional with current NMC/HCPC registration.
  • Evidence of continuing professional development.
  • Evidence of specialist clinical knowledge relevant to the role.
  • Minimum 3 years' post-registration experience.
  • Knowledge of current health and social care policies.
  • Knowledge of safeguarding and child protection procedures.
  • Experience of child and family-centred care.
  • Demonstrable clinical engagement and assessment skills.

  • Ability to travel throughout Wiltshire, Bath & North East Somerset and Swindon.
Desirable Criteria

  • Experience supporting complex or multi-agency cases.
  • Service evaluation, audit or quality improvement experience.
  • Teaching or mentorship experience.

The service operates on a rota basis, and the successful candidate will be required to work either:
  • 8:00am to 4:00pm, or
  • 10:00am to 6:00pm
