You will collaborate with PCPs and specialists to review plans, ensure documentation compliance, educate patients, and support daily management with sound clinical judgment.

Qualifications

• Associate's degree in Nursing or Nursing Diploma required with RN licensure.
• BLS certification; 2–5 years of nursing experience, bachelor's degree and 5–10 years preferred.
• Ability to assess patient needs, educate patients, and follow clinical protocols.
