Job summary

Elara Caring seeks a Clinical Team Manager to oversee patient care services, staff assignments, and care coordination across Home Health operations.

You will ensure timely initiation of care, compliance with regulatory guidelines, and review Plans of Care, physician orders, and changes for clinical appropriateness.

The role includes directing OASIS reviews, coaching staff, and partnering with leadership to monitor KPIs and clinical operations.

Qualifications

• Associate degree in nursing or related field required or preferred.
• 2 years of home care experience in Home Health or Hospice.
• 1 year of supervisory experience required.
• Current, unrestricted RN license valid for the state of work.
• Ability to quickly become proficient in Homecare Homebase and Workday; good Microsoft Office skills.

• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.

What is Required?

• Associate degree in a Nursing related field, Bachelor preferred
• 2 years of home care experience in Home Health or Hospice environment
• 1 year of supervisory experience required
• Current, unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
