Qualifications

• Minimum of 3 years of experience with behavioral health, substance use, medical, or complex social needs.
• Experience supervising or mentoring staff in community-based or behavioral health settings.
• Strong clinical assessment, crisis intervention, and care coordination skills.
• Excellent communication, organization, and interdisciplinary collaboration abilities.
• Ability to work independently and manage competing priorities in a fast-paced environment.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Provides clinical supervision and coaching to Community Health Workers and other staff.
• Reviews referrals to determine program fit and level of care.
• Supports timely connections to internal services and community resources.
• Coordinates referrals to external providers when needed.
• Identifies urgent clinical or safety concerns requiring escalation.
• Develops relationships with hospitals, health plans, providers, shelters, and agencies.
• Monitors referral workflows, intake timelines, and engagement outcomes.
• Participates in interdisciplinary case reviews and conferencing.
• Supports staff engagement strategies and crisis intervention approaches.
• Ensures services comply with MassHealth, contracts, and regulatory requirements.
• Reviews documentation for quality, compliance and clinical appropriateness.
