Job summary

Enhabit Home Health & Hospice is seeking an experienced nurse leader to oversee clinical operations for home health services. The role emphasizes coordination of care, staff supervision, and ensuring adherence to standards.

Ideal candidates bring active RN licensure, a minimum of two years of recent nursing experience with at least one year in home health, and strong communication skills to engage physicians, patients, and caregivers.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid RN license per state requirements or compact license.
• Experience as a licensed professional; home health experience preferred.
• Minimum 2 years nursing experience within the last 36 months.
• Minimum 1 year home health experience.
• Knowledge of federal/state/local regulations governing home care.
• CPR certification.
• Valid state driver's license and automobile liability insurance.
• Basic technology skills including mobile devices.
• Preferred: management experience.
• Preferred: bilingual English/Spanish.
• Must have dependable transportation and be able to drive in all weather.

Responsibilities

• Coordinate home health services delivered to patients per standards and procedures.
• Review and approve start of care, recertification, resumption of care, or evaluation visit information.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain communication to ensure care continuity.

Responsibilities

Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures. Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit. Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees. Ensure scheduling of patients to field clinicians and communication with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.

Qualifications

• Required graduate of an approved school of professional nursing.
• Required to have and maintain a valid clinical license, per state requirements, or in accordance with the board of nursing rules for a compact nurse license.
  • Experience as a licensed professional may be deferred with a review and approval from the vice president operations or regional president
• Required to have previous experience practicing under the professional license for this position.
• Required to have minimum of two years of nursing experience within the immediate previous 36 months.
• Required to have minimum of one year of experience with home health.
• Required to have demonstrated knowledge and understanding of federal, state and local laws and regulatory guidelines that govern the operation of a home care office.
• Required to have and maintain CPR certification, as required by position.
• Required to have and maintain a valid state driver license in the current state of residence
• Required to have and maintain automobile liability insurance, as required by law
• Required to have basic demonstrated technology skills, including operation of a mobile device.
• Preferred previous experience in a management position.
• Preferred previous experience using Homecare Homebase (HCHB).
• Preferred bilingual (English/Spanish) with the ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
• Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
• Oral communication, written communication, fluency in English, active listening.
• Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
• Other than for designated full-time remote positions, in-office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
