Job summary

Rock River Hospice & Home is seeking a full-time nursing professional to lead and manage Nursing and CNA staff. Candidates must hold an Illinois Nursing Licensure and possess at least two years of hospice or home care experience. Responsibilities include overseeing care delivery and maintaining relationships with healthcare providers.

Responsibilities

• Monitors all Nursing and CNA staff to ensure that planning and delivery of quality, cost‑effective hospice care is provided through scheduling efforts that reflect both high and low volume.
• Take on‑call, as required.
• Spend at least two days weekly as a patient care provider.

Community Interaction

• Help to maintain ongoing physician, nursing home, and hospital relationships.
• Provide education to groups seeking information about hospice and the hospice philosophy.
