Job summary

Stormont Vail Health in Topeka, KS is seeking a licensed Pharmacist to review medication orders, provide clinical interventions, and support the pharmacy team across a high-volume hospital setting.

Qualifications

Responsibilities

  • Review, interpret and verify provider medication orders making any recommendations and clinical interventions as necessary based on patient specific condition, disease states, age and other clinical factors such as safety and cost effectiveness.
  • Complete pharmacy consults proficiently, following pharmacy policies and procedures. Provide drug information to healthcare professionals and education to patients by using interpersonal skills to effectively communicate and interact with all members of the healthcare team while multitasking to ensure all assigned tasks are completed.
  • Facilitate completion of pharmacy medication distribution functions. Examples include directing pharmacy technician workflow and verification of accuracy of all final products prior to dispensing to patients.
  • Manage medication use including compounding, preparation, selection, dispensing, storage and stocking of medications for the entire organization.
  • Collaborate with pharmacy team to ensure provision of optimal patient care. Capability to maintain positive relationships with coworkers and social awareness. Educate and train pharmacy personnel and other healthcare professionals and serve as preceptors for pharmacy students and pharmacy residents.
  • Develop and conduct medication use evaluations as needed, and report results to the relevant committees or subcommittees to implement in daily practice.
  • Design, implement, and lead pharmacy process improvement initiatives, including those focused on evidenced based care, safety, financial sustainability or quality improvement processes.
  • Address and complete provider consults proficiently and provide drug information to healthcare professionals and education to patients.
  • Educate and train pharmacy personnel and other healthcare professionals and serve as preceptors for pharmacy students and pharmacy residents.

Education Qualifications
  • Bachelor’s Degree of Science in Pharmacy from an accredited school of pharmacy. Required or
  • Doctorate PharmD from an accredited school of pharmacy.
Experience Qualifications
  • 1 year Pharmacy practice experience as a licensed Pharmacist or successful completion of a Post Graduate-Year 1 accredited pharmacy practice residency. Preferred
Skills and Abilities
  • Medical terminology and knowledge of medication names and how to effectively run a pharmacy
  • Ability to apply mathematical concepts such as exponents, logarithms, quadratic equations, and permutations. Ability to apply mathematical operations to such tasks as frequency distribution, determination of test reliability and validity, analysis of variance, correlation techniques, sampling theory, and factor analysis.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
  • Spreadsheets, Internet/E-mail, Database software, Contact Management, Electronic Medical Record
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Licenses and Certifications
  • Pharmacist - KBP Required within 90 days of hire. Required
  • Basic Life Support - BLS Required within 90 days of hire.
  • Advanced Cardiovascular Life Support - ACLS Required within 90 days of hire.
