Reviews prescriptions and medication orders for appropriateness and compatibility with current therapy, disease states, drug allergies, adverse drug effects, duplication of therapy and compliance to ensure patients' medication therapy is safe, effective, timely, and free from drug-related problems. Communicates with all healthcare professionals and patients concerning medical care to ensure safety and effectiveness.
