Job summary

Humboldt Park Health is a community-based hospital dedicated to serving the Humboldt Park community with patient-centered care and a strong focus on quality. We seek a licensed pharmacist to lead medication management, oversee pharmacy operations, and ensure compliance with all state and federal regulations.

The role emphasizes accurate medication profiling, collaboration with healthcare providers, and participation in QA/PI initiatives to continually improve patient safety and outcomes.

Qualifications

  • Graduate of an ACPE accredited college of pharmacy.
  • Pharmacist licensure required.

Responsibilities

  • Reviews, processes, and modifies, as necessary, all medication orders to maintain a current and accurate medication profile.
  • Supervises, directs and reviews all activities involved with the compounding, storing, inventory, labeling, recording, or preparing medications by pharmacy technicians.
  • Monitors drug therapies to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic levels, drug interactions, and therapeutic duplication.
  • Monitors the use of drugs for compliance with prescribing criteria, guidelines, formulary, and protocols established by Norwegian American Hospital.
  • Detects, monitors, documents and reports adverse drug reactions and medication errors.
  • Serves as a resource of drug information for all practitioners and patients (i.e. drug identification, side effects, adverse events, interactions, medication counseling, etc.).
  • Participates in pharmacy quality assessment/performance improvement (QA/PI) projects and/or initiatives.
  • Assures proper control, documentation, and record keeping of controlled substances.
  • Ensures all state and federal regulatory requirements are maintained.
