Job summary

Evolve Therapy Services seeks a Speech Language Pathologist to join our in-house SNF in Pahokee, FL. You will plan, direct, and provide speech therapy services in accordance with federal, state, and local standards.

Requirements include a Master’s degree in SLP, CCC, active license in the state, and ongoing continuing education. This PRN role offers competitive pay and opportunities for growth within the organization.

Qualifications

• Master's degree in Speech Language Pathology required.
• Certificate of Clinical Competency required.
• Must have an active SLP license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide quality speech therapy services and initiate referrals when appropriate.
• Evaluate and obtain data necessary for treatment planning and implementation.
• Develop a treatment plan with target list, short-term, and long-term goals.
• Instruct patient, family, and staff in appropriate speech-language procedures to carry over goals.
• Complete CMS-compliant documentation in NetHealth EMR systems.
• Oversee FEES/MBS protocols and coordinate site programs.
• Participate in screening protocols to meet resident needs timely and with clinical excellence.
