Qualifications

• Masters Degree in Speech Pathology from an accredited program.
• Wisconsin or Minnesota state license to practice as SLP.
• Active ASHA membership or CCC-Clinical Competence eligibility.

Responsibilities

• Provide screenings, evaluations and direct therapy to clients in a 1:1 medical model.
• Manage client medical records to assist in determining evaluation and treatment plan.
• Evaluate patients to identify strengths and deficits.
• Determine treatment plan, long-term goals, short-term goals, and duration of treatment.
• Serve as a member of the interdisciplinary team to develop a comprehensive treatment plan.
• Maintain expected utilization and productivity requirements.
• Communicate effectively with care providers and family to ensure accurate treatment planning.
