Job summary

Tender Touch Rehab Services LLC is seeking a Physical Therapist – Clinical Specialist for a full-time position in Naugatuck, CT. The setting is Skilled Nursing Facility / Long-Term Care, with compensation of $48.00–$55.00 per hour.

You will evaluate and treat patients, develop personalized care plans, and ensure regulatory documentation and physician orders are up to date. Must be a licensed Physical Therapist in the state, with SNF experience and willingness to travel between facilities.

Qualifications

• Degree from an APTA-accredited Physical Therapy program (BS, MS, or DPT).
• Current license as Physical Therapist in the state of practice.
• One year of SNF experience and willingness to travel to multiple facilities.

Responsibilities

• Evaluate and treat patients with mobility, strength, balance and functional impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
