Qualifications

• Master's degree in Social Work required.
• NY LMSW or LCSW license required; eligible for Medicaid/Medicare.
• Experience with population served by the clinic; therapy, assessments, care coordination preferred.
• Must be authorized to work in the U.S.; may require a NY driver’s license.

Responsibilities

• Conduct comprehensive psychosocial assessments through interviews and record reviews.
• Provide individual, group, and family therapy.
• Develop and modify individualized treatment plans.
• Coordinate referrals and ensure continuity of care across services.
• Provide crisis intervention when needed and participate in discharge planning.
• Supervise social work students when assigned.
