Job summary

Qualifications

• Master’s degree in social work (MSW) from an accredited school of social work.
• Knowledge of CPS and APS mandatory reporting laws.
• Two years’ experience in social services and/or discharge planning in post-acute care preferred.
• Knowledge of basic medical terminology and understanding of disease impact on patients and families.
• Advanced communication skills with conflict resolution capabilities.

Responsibilities

• Completes psychosocial assessments of patients to evaluate patient needs.
• Supports patients and families to cope with hospitalization and new diagnosis.
• Provides crisis interventions, grief/bereavement counseling, and facilitates family meetings with reports.
• Collaborates with care providers to develop treatment recommendations.
• Creates and updates a database of home and community resources for the team.
