Job summary

Duke Health's Duke Raleigh Hospital in Raleigh, NC seeks a Clinical Social Worker to deliver psychosocial assessments, treatment planning and discharge planning for patients and families facing hospitalization and illness. Level I functions independently; Level II provides supervision and leads team initiatives.

The role collaborates with a multidisciplinary team, adheres to NASW standards, and participates in quality improvement activities.

Qualifications

• Master’s degree in Social Work from an accredited school.
• Current LCSW credential in NC required.
• Two years of post-internship healthcare social work experience and two years post-master’s clinical experience preferred (Level I/II).

Responsibilities

• Conduct psychosocial assessments and develop treatment plans.
• Provide crisis intervention and therapy for patients and families.
• Coordinate pre/post-hospital services and community resources.
• Lead team meetings and supervise Level II clinicians.
• Document encounters and maintain records per standards.
