Job summary

Qualifications

• Experience with regulatory reporting of child/elder abuse and domestic violence.

Responsibilities

• Interview and evaluate patients and families to identify social, emotional, and economic factors.
• Develop and implement social work treatment plans for patients and families.
• Coordinate with other professionals and community resources; refer as needed.
• Provide psycho-social therapy and participate in multidisciplinary health team meetings.
