RiverMead, a LifePlan Community in the Monadnock Region, seeks a Clinical Social Worker to assess, counsel, and advocate for residents and families across Independent Living, Assisted Living, Memory Care, and hospice. You will collaborate with an interdisciplinary team to keep care centered on each person.

Required: MSW from CSWE-accredited program, NH social work license, and 2+ years in health care or long-term care for older adults.

Qualifikationen

• Master's degree in Social Work (MSW) from a CSWE-accredited program.
• Current New Hampshire social work license.
• At least two years of social work experience in health care or long-term care for older adults.
• Knowledge of aging, dementia, and healthcare systems.
• Experience with psychosocial assessment, counseling, and care planning.

Aufgaben

• Provide psychosocial assessment, counseling, care coordination, and advocacy for residents and families across all care levels.
• Maintain regular office hours and drop-in times to be accessible to residents and families.
• Identify emotional, social, and environmental needs and implement support plans.
• Educate and provide resources on dementia, end of life, loss, anxiety, and depression.
• Collaborate with Care Coordination Team to ensure smooth transitions between levels of care.
