Clinical Social Worker, CF

Under the general direction of the Supervising Psychiatric Social Worker, CF, the Clinical Social Worker (Health/CF) Safety conducts assessments and summarizes case information to use in diagnosis, treatment, and dispositional release; diagnoses and/or collaborates in the formulation of a diagnosis; develops, monitors, and modifies treatment plans in collaboration with the interdisciplinary treatment team.

Additional duties include providing assessments, group therapy, and collateral conferences, and documenting and evaluating patients’ progress in treatment.

Key responsibilities include:

• Conducting comprehensive assessments and summarizing case information for diagnostic and treatment purposes.
• Collaborating with the interdisciplinary team to formulate diagnoses and treatment plans.
• Monitoring and adjusting treatment plans in a dynamic, patient‑centered manner.
• Providing group therapy sessions and facilitating collateral conferences.
• Documenting and evaluating patient progress and outcomes.

Authorized HAM rate: P $8,678, Q $9,319.
