Qualifications

• Master of Social Work (MSW) is required.
• Two years of recent health-care environment experience is required.
• Licensed Clinical Social Worker (LCSW) is required; BLS certification may be required depending on setting.

Responsibilities

• Provide social work services to patients and families to meet psychosocial and clinical needs.
• Identify and focus on patients needing social work support to align care.
• Analyze information from patients and staff to understand situations and prevent gaps.
• Link patients with community resources to ensure access to care.
• Craft care plans with minimal supervision.
• Educate staff, patients and families as a liaison on care information.
• Provide crisis support and leadership in high-tension situations.
• Complete psychosocial assessments and link patients to appropriate care.
• Utilize counseling and therapy techniques to ease patients and families.
• Perform other duties as assigned.
