Job summary

Cone Health Medical Group is seeking a Clinical Social Worker II to provide psychosocial and clinical services for patients and families. The role uses therapeutic techniques to identify needs and connect patients with appropriate care, inside or outside the clinical setting.

The position requires an MSW, a minimum of two years in a health-related environment, and licensure as a Clinical Social Worker with BLS certification.

Qualifications

• Master of Social Work (MSW) is required.
• Minimum of two years of recent experience in a health-related environment.
• Licensed Clinical Social Worker is required; BLS certification is required.

Responsibilities

• Determines which patients to focus clinical social work efforts on to help staff and patients alike be in the correct environment with appropriate social work support.
• Analyzes information from patients and clinical staff to understand the situation and communicates it to all parties to prevent issues from information gaps.
• Links patients with community resources and health opportunities based on clinical knowledge to ensure access to quality care.
• Crafts a care plan for patients with minimal assistance to enable right treatment options.
• Educates staff, patients and families about critical information related to care to help all understand the situation as liaison.
• Provides crisis support and leadership in high-tension situations for staff, family and patients using clinical expertise to de-escalate worries.
• Provides clinical social work services to patients and families to meet psychosocial and medical needs.
• Completes psychosocial assessments to link patients with suitable care options based on medical and social status.
• Utilizes counseling and therapy techniques to ease patients and families during clinical journeys.
• Performs other duties as assigned.
