Job summary

Wentworth-Douglass Hospital is seeking a Part-Time Clinical Social Worker to support the ED, inpatient, and palliative care teams. The role collaborates with patients, families, and healthcare professionals to assess psychosocial needs and develop patient-centered treatment plans.

Schedule is 24 hours/week, days, 8:00 AM–4:30 PM, with weekend and holiday rotation. Minimum education: Master's in Social Work or Mental Health & Behavioral Medicine; NH Clinical Social Worker license required.

Qualifications

  • Master's Degree in Social Work or Mental Health & Behavioral Medicine required.
  • NH Clinical Social Worker License required.
  • Hospital experience preferred.

Responsibilities

  • Collaborate with patients, families, and healthcare professionals to evaluate patients’ medical and psychosocial needs.
  • Develop and implement individualized treatment plans that support patient-centered care across ED, inpatient, and palliative care settings.
  • Coordinate documentation and ensure appropriate communication within the interdisciplinary team.
