Job summary

Legacy Health in Oregon is seeking a Clinical Social Worker I or II to provide psychosocial assessments and crisis intervention across adult and pediatric settings, focusing on vulnerable populations and care coordination. You will collaborate in a multidisciplinary team, supporting transition planning and end-of-life decisions, while navigating EMR systems.

Qualifications

• Master of Social Work degree from an accredited School of Social Work required.
• Experience: 1 year preferred in care management, mental health or addictions services, medical social work, or social service organization.
• For II: two years of relevant experience required; licensure paths noted.

Responsibilities

• Provides social work services in adult and pediatric settings with a focus on holistic patient-centered care.
• Works independently and within a multidisciplinary team.
• Performs psychosocial and mental health assessments, crisis management, and transition planning; coordinates internal and external services.
• Addresses behavioral health, addiction issues, end-of-life care, and informed decision making with surrogates and families.
• Educates and facilitates support or treatment groups as needed.
• Aims to enhance patients' and families' ability to meet their own needs.
• Non-exempt position with variable hours and schedules.
