Job summary

California Correctional Health Care Services seeks a Clinical Social Worker (Health/CF) Safety at Folsom State Prison to assess patients, develop treatment plans, and support safer, humane care within a correctional setting.

The role includes providing group therapy and collateral conferences, coordinating placement with community resources, and monitoring patient progress with the interdisciplinary team to support safe, humane reintegration and ongoing care for individuals under supervision.

Responsibilities

• Conduct assessments and summarize case information to use in diagnosis, treatment (level of care), and dispositional release.
• Develop, monitor, and modify treatment plans in collaboration with the interdisciplinary treatment team.
• Provide assessments, group therapy, and collateral conferences, and document and evaluate patients’ progress in treatment.
• Coordinate placement with community resources and the Classification and Parole Representative.
