Overview

The Clinical Social Worker (CSW III) provides psychosocial assessment and intervention to kidney transplant patients and families with complex psychosocial needs, collaborates with multidisciplinary teams to support safe discharge and ongoing care, and serves as a liaison to community resources.

Responsibilities
  • Conduct psychosocial assessments and interventions based on risk screening, evaluating prior functioning levels, support systems, and the patient’s ability to cope.
  • Provide crisis intervention and counseling to transplant patients, families, and other patients with complex dynamics.
  • Act as primary care manager for patients with significant psychosocial needs, managing clinical resources and transition planning from admission through discharge.
  • Meet with provider, care team, and patient to set medical readiness goals and formulate primary and backup discharge plans.
  • Coordinate and lead multidisciplinary rounds, monitor discharge milestones, assign accountability for tasks, and proactively identify and remove barriers to timely discharge.
  • Reassess patients as needed to adjust discharge plans in real time.
  • Maintain complete and accurate documentation in APeX screens and patient records.
  • Participate in discharge planning activities for complex patients, driving timely discharge and linking to post‑discharge care.
  • Educate patients, families, physicians, and community members about post‑acute options and end‑of‑life issues.
  • Address domestic violence, child/elder abuse, and other reportable situations in accordance with legal mandates and UCSF policy.
  • Collaborate with psychiatry and other teams on psychiatric holds, mental health or substance abuse problems, and suicidal or homicidal ideation.
  • Provide counseling related to treatment decisions and end‑of‑life care, including advance health care directives and POLST completion.
  • Ensure timely submission of documentation and statistical data in compliance with department standards.
  • Attend department, hospital, and community in‑services, quality improvement activities, and program planning.
  • Supervise and provide clinical supervision to CSW II staff for LCSW licensure in accordance with the California Board of Behavioral Sciences.
  • Cover assigned shifts and duties as needed.
  • Maintain collaboration and communication with the multidisciplinary transplant team in evaluation, listing, and management of pre‑ and post‑transplant patients.
  • Develop educational materials and facilitate transplant education seminars and support groups.
Qualifications
  • Master’s degree from a CSWE accredited School of Social Work.
  • Experience (internship or career) in a healthcare and/or hospital setting preferred.
  • Strong knowledge of community resources pertinent to illness, symptoms, mental status, and functional impairments.
  • Understanding of the medical and psychological sequelae of disease and their impact on social functioning.
  • Demonstrated resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
  • Self‑directed, assertive, and creative problem‑solving and systems planning in a high‑volume environment.
  • Ability to work effectively and collaboratively with interdisciplinary teams.
  • Basic pager, telephone, and computer skills including internet use.
  • Flexibility to orient and work at all UCSF Medical Center locations.
Additional Requirements for CSW III
  • Licensed Clinical Social Worker in the state of California, Board of Behavioral Sciences.
  • Minimum three years of experience in a healthcare setting, with hospital experience preferred.
  • Leadership experience in advanced program planning, educational programming, clinical supervision, and consultation to the clinical team and community.
Preferred Qualifications
  • Current California state license in good standing.
  • Three years of healthcare experience, preferably in a hospital setting.
Licenses & Certifications
  • Licensed Clinical Social Worker (LCSW) in California, Board of Behavioral Sciences.
