Family Health Centers of San Diego is seeking a Clinical Site Manager, Physical Therapist to oversee Clinical Site Operations and provide direct patient care as needed. The role focuses on administrative and clinical oversight across Physical Rehabilitation Services and related clinics.

You will lead staff, ensure adherence to licensure, regulatory standards, and documentation quality while driving performance and patient outcomes in a safety-conscious, community-focused environment.

Qualifications

• Master’s Degree in Physical Therapy is required.
• 3 years of leadership experience in healthcare are required.
• 2 years of supervising/leading in a clinical setting are required.
• Current California licensure as a Physical Therapist; valid CPR/BLS/ACLS credentials.

Responsibilities

• Oversee day-to-day operations of assigned Physical Rehabilitation Services clinics.
• Provide direct patient care as a PT clinician up to 60% time.
• Develop and implement plans to meet tactical and strategic objectives.
• Monitor site productivity, revenue, expenses and regulatory compliance.
• Collaborate with Senior Management to plan improvements and Lean initiatives.

We are also the largest health care safety-net provider, largest school-based health care provider and the largest mental health provider in the San Diego region.Family Health Centers of San Diego is looking for a Clinical Site Manager, Physical Therapist who provides operational management to include administrative and clinical oversight to Physical Rehabilitation Services and activities at assigned clinics. Will dedicate 60% of time providing direct patient care as a Physical Rehabilitation Services clinician. Actively demonstrate passion in providing caring, high-quality care to a diverse patient population with a special commitment to the uninsured, low income, and the medically underserved.Responsibilities:Build positive relationships, inspire others to perform at a higher level, prepare career development plans and retain an accountable workforce.Coach and counsel staff and administer appropriate disciplinary action in a timely manner.Provide/ensure staff receive all new hire and annual trainings on time.Administer day-to-day operations of assigned clinics, and/or assigned programs to include scheduling and patient service management.Establish operational objectives, and develop and implement plans for meeting tactical and strategic objectives as reviewed by senior management.Oversee administration of integrated care activities at assigned clinics to include data collection and report generation. Track site productivity and utilization rates. Monitor on-going revenue and expenses. Collect and analyze data, prepare necessary reports, and develop improvement plans as necessary.Provide direct patient services as appropriate within scope of licensure.In collaboration Physical Rehabilitation Services Senior Management Team, actively participate in organizational short and long-term planning and implementation activities.Oversee and directly participate in project planning, implementation, and assessment at assigned clinics.Complete all required documentation accurately, in a timely and thorough manner, in accordance with department standards.Ensure documentation complies with patients insurance requirements. In addition, monitor documentation of direct reports and assigned clinics to ensure maximum revenue realization.Work effectively as member of a multi-disciplinary treatment team.Communicate effectively with providers, physicians, case managers, scheduling staff, and department leadership in a timely and appropriate manner.Review and maintain clinical guidelines.Assess quality of clinical care.Participate in clinical peer review.Advise on standards of care for clinical practice.Ensure applicable regulatory and department standards are met or exceeded on an ongoing basis (i.e., JCAHO, OSHA, Safety Standards, etc.).Ensure assigned clinics and/or programs are managed in a fiscally responsible manner to include grants implementation and requirements as directed in collaboration with other FHCSD departments and clinics as applicable.Ensure assigned site is well maintained and in compliance with FHCSD policies.Ensure work collaboratively with other managers in addressing and servicing difficult patients.Ensure Physical Rehabilitation Services staff are kept informed and adhere to policies and procedures that relate to the Physical Rehabilitation Services clinic.Participate in continuous improvement activities and ensure appropriate Lean tools (i.e., 5S, A3) are deployed and results are evaluated and/or measured.Perform other duties as assigned.Requirements:Master’s Degree of Physical Therapy required.3 years leadership experience in Healthcare required.2 years experience leading or supervising in Clinical setting required.Current California state clinical licensure as Physical Therapist.Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training issued by an authorized agency.Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency.
