Health, Inc. is seeking a Clinical Registered Nurse to provide high-quality, person-centered care to seniors in skilled nursing and rehab settings. The role includes assessments, medication administration, and collaboration with an interdisciplinary team.

You will educate residents and families on health practices, document activities in the EHR, and mentor CNAs while contributing to quality improvement and infection control initiatives.

Qualifications

• Current registered nurse licensure
• Compassionate and person-centered approach to care
• Strong communication and teamwork skills
• Ability to educate and support residents and families
• Experience with electronic health records (EHR)

Responsabilités

• Provide high-quality, person-centered care to seniors in skilled nursing and rehab settings
• Perform assessments, administer medications and treatments
• Coordinate individualized care plans in collaboration with an interdisciplinary team
• Educate residents and families on health and care practices
• Document care activities accurately in the EHR
• Support residents' spiritual and emotional needs within a faith-based environment
• Mentor Certified Nursing Assistants (CNAs)
• Participate in quality improvement and infection control initiatives
