PACE Southeast Michigan is seeking a Clinic RN Coordinator in Eastpointe, MI to lead clinical operations and coordinate nursing staff in delivering high-quality care to participants.

The role requires BSN preferred, experience with frail elderly, and strong leadership and communication skills to guide LPNs, medical assistants, and scheduling clerks while ensuring adherence to quality standards and care transitions.

Qualifications

• BSN preferred; licensed RN.
• Minimum one year experience with frail elderly.
• Five years of management experience preferred.
• Ability to lead and direct staff and coordinate care.

Responsabilités

• Collaborates with physician team to ensure optimal clinical flow.
• Oversees daily clinical workflow and task delegation to LPNs, MAs, and scheduling clerks.
• Triages participants and provides educational resources.
• Supervises staff to ensure competency and adherence to quality standards.
• Ensures documentation in participants' medical records daily.
• Collaborates with RN Case Managers on transitions of care.
