Job Summary

Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice.

Essential Functions
  • Provides direct patient care in accordance with current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator and Charge Nurse.
  • Provides and supervises community activities according to needs.
  • May include driving & transporting patients to and from activities in hospital vehicles.
  • Remains awake and alert, at all times, while on the job.
  • Assists patients in maintaining and keeping various appointments.
  • Clear, concise, and timely documentation and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourages patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Physical ability for participation in patient activities.
  • Other duties as assigned.
Qualifications
  • Education: High School Diploma or Equivalent required; Bachelor's Degree Psychology preferred; Bachelor's Degree Social Work preferred; Bachelor's Degree Health & Human Services preferred; Associate's Degree Related Field of Study preferred.
  • Licenses and Credentials: Medication Administration Program Certified (MAP - Massachusetts) – Massachusetts Department of Public Health preferred.
  • Experience: Experience working in a human services, residential, behavioral health, or allied health setting 1-2 years required; Experience working with individuals with mental illness, substance use disorders, or developmental disabilities 1-2 years required.
  • Knowledge, Skills and Abilities: Basic knowledge of behavioral health conditions and support strategies; Strong interpersonal and communication skills with the ability to maintain professional boundaries; Ability to manage time, multitask, and respond to urgent needs calmly and effectively; Commitment to person‑centered care, equity, and cultural competence; Familiarity with safety protocols and documentation standards in healthcare or social service environments.
