Sweetwater Care is seeking a licensed physical therapist to provide patient-centered care in our Fresno clinic. You will assess patients, design individualized treatment plans, and guide therapy sessions that promote healing and functional gains.

Qualifications

• Clinical judgment and assessment skills
• Ability to develop and monitor individualized treatment plans
• Proficient in documenting patient progress and outcomes
• Strong communication with patients, families, and healthcare teams
• Supervision of support staff such as assistants and aides
• Licensure in the state of California (Active State Licensure)

Responsibilities

• Consult with patients to learn about their physical condition and symptoms and review medical history.
• Diagnose movement dysfunction by observing patients and listening to their reports.
• Develop individualized treatment plans to address impairments and functional goals.
• Coach patients and teach therapeutic massage techniques to ease pain and promote healing.
• Maintain comprehensive patient records including goals, progress, and regulatory requirements.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per regulatory requirements.
• Evaluate outcomes and modify services based on patient response and referrals.
• Provide counseling to patients and families about disorders and treatment options.
• Communicate clearly with supervisors and health team members about progress and plans.
• Provide comprehensive discharge summaries in line with regulatory requirements.
• Document all regulatory and clinical information in the electronic system and maintain records as required.
• Participate in trainings and conferences as required by the facility and company.
• Possible supervision of physical therapist assistants and rehabilitation aides as needed.
• Adhere to company policies, procedures, and code of conduct.
