Job summary

The role provides coverage across Kingston and Carmel, delivering individualized therapy programs, assessments, treatment plans, and patient education to maximize progress toward functional goals.

The ideal candidate has a Master’s Degree in Physical Therapy, NY State license, and experience with Electronic Health Records. Some travel within the Kingston area is required.

Qualifications

• Master’s Degree in Physical Therapy required.
• New York State license required.
• Experience with documentation in Electronic Health Records preferred.

Responsibilities

• Provide direct patient care and evaluate functional needs and outcomes.
• Design and implement a plan of care based on assessments.
