Job summary

Martin's Point Health Care in Maine seeks a Clinical Program Manager - Case Management to lead strategic initiatives for Generations Advantage and USFHP, designing and overseeing targeted programs that improve care transitions, reduce avoidable readmissions, and enhance post-discharge follow-up within regulatory requirements.

Qualifications

• Bachelor's degree in Health Science, Nursing or related field required.
• Master's degree in Business Administration (MBA), Healthcare Administration (MHA), or Public Health (MPH) preferred.
• Certification as CCM or ACMA preferred.
• 10 years’ experience in healthcare including managed care and program management.
• Experience with data analysis, ROI measurement, and monitoring program performance.

Responsibilities

• Supports program development activities, including operational reporting and program resources.
• Translates organizational strategy into actionable, localized case management work plans.
• Streamlines case management workflows to improve efficiency and member engagement.
• Optimizes data reporting structures to track program metrics and identify trends needing intervention.
• Monitors KPIs to measure program success and ensure regulatory compliance.
• Develops presentations, communications, and program tools for CM Program.
• Partners with cross-functional leaders to align care models and initiatives.
• Facilitates committees and working groups to communicate updates and outcomes.
• Collaborates with IT and analytics to refine case management platforms and dashboards.
