Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking five (5) Clinical Practice Transformation Specialists for the Rural Health Transformation (RHT) Program.
The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Essential Functions:
Desired Education: Four years post-secondary schooling
Desired Degree: Bachelor's degree
Desired Major: Nursing, allied health or related discipline
Desired Job Experience:3 to 5 years prior clinical experience. Equivalent clinical experience may be substituted for degree requirement.
Other Qualifications: Strong teamwork and interpersonal skills as well as the ability to effectively communicate at all levels. Good time management and organizational skills along with the ability to manage continually changing priorities. Ability to exercise independent and sound judgment in decision making. Experience with Statistical Research Methods and Advanced Healthcare or Nursing Research required. Excellent oral and written communication skills. Strong computer and keyboard skills. Must hold a valid and unrestricted motor vehicle operator’s license and provide documentation of required level of automobile insurance coverage. Must have reliable transportation.
Certifications Required: State Motor Vehicle License
